Thyroid follicular adenoma

  • Commonest cause of solitary thyroid nodule
  • Solid mass with fibrous capsule - normally non functioning
  • Wide age range but F > M

 

Thyroid follicular carcinoma

  • Well differentiated malignant tumour with good prognosis
  • 20% of carcinomas
  • 90% solitary nodule 10% with mets

 

Papillary thyroid carcinoma

  • Well differentiated carcinoma with excellent prognosis. Younger mostly female
  • Most common carcinoma
  • Invades lymphatics
  • Annie eye cuclei under microscopy

Papillary Prognosis is Perfect
Anaplastic is Awful (not treatment responsive usually)
Medullary = MEN2, with most common presentation of hypercalcaemia therefore calcitonin-secreting

The presence of lymph node metastasis contrasts papillary thyroid cancer with follicular thyroid cancer, where haematological metastasis predominates.
