Drug Class Quinolone
Mechanism Broad spec bactericidal abx - inhibits DNA gyrase (needed to coil and replicate bacterial DNA)
Typical Dose 500mg BD
Indications
  • Gram -ve infections (Salmonella, pseudomonas, campylobacter, neisseria, e coli, h. influenzae
  • Some gram +ve
  • Fistulating Crohn's disease
  • Acute otitis externa 
  • Superficial bacterial eye infection (severe)
  • Corneal ulcer
  • RTIs
  • Acute prostatitis
    • Surgical prophylaxis
Contra-indications

Pregnancy

Children

Caution: epilepsy (lowers seizure threshold)

Side-effects

Common: common ones

Corneal deposits (reversible after completion of treatment)

Arthropathy in children

Interactions
  • Cyclosporin: ↑ risk of nephrotoxicity
  • Theophylline: ↑ risk of convulsion
  • NSAIDs: ↑ risk of convulsion
  • Warfarin: ↑ antocoagulant effects
Elimination  
Additional Notes Clinically used in UTIs, resp and GI tract infections, gonorrhea and septicaemia.
